By electing to participate in the Mayne Pharma Medicare Part D Alternative Coupon Program you will need to opt-out of using your Medicare Part D prescription plan for Imvexxy® and/or Bijuva® and agree not to seek any prescription coverage or reimbursement from your insurer for the cost of the product or report any amounts paid for the product as part of your true out-of-pocket expenses under Medicare Part D within this calendar year.
